Community Support Service

Linkage's Community Support Service has been established to provide support and general counselling to adults with learning disabilities within their own accommodation.

The Community Support Service aims to assist all people to continue to develop and maximise their existing abilities to maintain and enhance their independence. Care will be taken to risk assess the physical environment in which they live.

Staff will provide support on an individualised formally agreed basis, which will be recorded in the individual support packages. Relevant support will be provided with undertaking routine day-to-day living tasks and appropriate training, personal care tasks, financial and administrative tasks, focusing on each person’s personal needs and choices identified in their support package.

The Community Support Service will primarily focus upon enabling people to integrate and gain independence through counselling and support.
